I work as a Neurology consultant at Enevia Health LLC, in the field of neurodevelopment.
Diagnoses such as Autism Spectrum Disorder or Attention Deficit are clinical diagnoses, that is, “symptom-based.” It is essential to study the reason why neurological development does not occur adequately. Under this paradigm, in precision medicine we evaluate immunology, not only to rule out known causes of encephalitis, but because when there is neuroinflammation (inflammation of the brain) the connections will not occur adequately. We know that a large percentage of children diagnosed with ASD have immunological alterations and that many concomitantly have repeated infections.
Allergies and eczema are common conditions that require root treatment, as the increase in histamine generates neuronal hyperexcitability with worsening of behavior, obsessions and restlessness.
On the other hand, we know the importance of the gut-brain axis, the microbiota works for proper neurodevelopment, intestinal dysbiosis causes or worsens neuroinflammation. The microbiota also produces substances important for neurological development such as short-chain fatty acids (SCFA), vitamins, activation of dietary substances, neurotransmitter metabolism and immune signaling. All of this must be measured in each child.
We also know that children with neurodevelopmental disorders are more susceptible to contaminants, heavy metals and mycotoxins, variables that we also study with precision tests.
The study of mitochondrial function is essential. We know that during neurodevelopment the brain consumes up to 40% of the total basal metabolism to grow and connect, and many children have mitochondrial problems that hinder this connection.
Assessing nutritional status with dosage of vitamins, minerals and critical nutrients is essential.
Precision medicine offers the possibility of studying and reversing all possible causes that may affect children's development, so that they can develop fully.
NEUROLOGY
Many children with a diagnosis of ASD, when studied, meet the criteria for PANDAS or PANS (Pediatric Autoimmune Neuropsychiatric Disorders Associated with Streptococci) and Pediatric Acute Onset Neuropsychiatric Syndrome.
Symptoms are often confused with Autism, OCD (obsessive), motor or vocal tics, mood swings, irritability, anxiety, regression of acquired maturation patterns.
What is behind this is an abnormal immune Rta that attacks specific places in the child’s brain due to "molecular mimicry" between some germs and the brain. This entity may be associated with other germs not only streptococcus, (Lyme, herpes, mycobacteria, parvovirus, influenza, etc.) that must be assessed with a complete immune status. Other triggers have also been described, such as environmental toxins, mycotoxins, heavy metals and chemicals. Comprehensive study of the child to identify and treat specifically is essential.
The literature suggests that PANDAS/PANS is an autoimmunity against D1R dopaminergic receptors that inflame basal ganglia in the brain (area responsible for motor control, cognitive functions, procedural learning and emotional regulation). This autoimmunity occurs in the context of genetic predisposition, history of infection (streptococcus), alteration of BBB (blood-brain barrier) and causes inactivation of the cholinergic interneurons of the basal ganglia and dysregulation of the claustrum.
It will be essential in case of suspicion to evaluate specific cytokines, markers of BHE permeability and neuroinflammation, the immune status of the child, possible germs involved and specific Acs for example against R. of Dopamine among others.
If all this is not evaluated and treated in an appropriate and precise way, it will fall under the clinical diagnosis of Autism, losing the opportunity to perform specific treatments.
